Anchor Checklist

General Information

Items to Check

1. Workers know appropriate anchorage points for each task

2. All anchorage points are stable, substantial, and have sufficient strength to withstand twice the potential impact energy of the free-fall

3. All anchorage points for body harnesses located at shoulder height

4. Anchorage points for self-retracting lifeline systems located overhead

5. A worker can move from one station to another or climb up and down without exposure to a fall.

6. The entire lifeline system has been designed and approved by a competent person

7. The anchorages to which the lifeline is attached has been designed and evaluated specifically for a lifeline

8. The rope or cable free from signs of wear, abrasion and or discoloration

9. The rope or cable have the required initial sag

10. Adequate Identification/Inspection labels displayed clearly

11. Connecting hardware is not damaged or in poor condition (e.g. unable to lock).
