Checks for Annual Testing of Traction Lifts (both Full and/or No-Load Test)
Status = Satisfactory (S); Not Satisfactory (NS); Not Applicable (NA)
MACHINE ROOM/HEAD ROOM
1. Traction motor condition (e.g. coupling condition)
2. Drive & traction and deflector sheaves (e.g. groove profile and condition)
3. Gearbox (e.g. oil condition and level, excessive backlash, bearings condition)
4. Effectiveness of brakes (checks include: free moving plunger, plunger lever movement, brake drum surface contamination, verification of air gap)
5. Overspeed governor sheave condition
6.1 Overspeed governor tripping mechanism
6.2 Overspeed governor overspeed switch function
6.3 Flyweights condition (e.g. free/non-restricted movement of flyweights)
6.4 All linkages and moving parts in the overspeed governor are free of defects
6.5 Overspeed governor gripping jaw function (e.g. jaw is able to effectively grip the rope, pulling force of the governor effectively engages the safety gear)
6.6 Safety gear switch function
6.7 Safety gear function (e.g. after the safety gear is engaged, the rope should slip as even if the traction machine is running)
7. Ascending Car Over speed Protection (ACOP):
8. Unintended Car Movement Prevention (UCMP) function
9. Terminal speed reduction system function
10. Emergency power supply for lighting, ventilation, alarm and intercom systems (e.g. EBOPS, ARES, UPS) function
11. Controller and electrical system (e.g. PCB delamination, signs of overheating, function of ELCB)
12. Governor rope Ø
13. Automatic Rescue Device (ARD) function
HOIST WAY
14. Door locks and switches (e.g. landing door, emergency access door)
15.Suspension rope condition (e.g. excessive broken wires of strands, excessive rouging, excessive rust; according to manufacturer's recommendations)
Fill in the measured Ø of each rope (mm) in the table below:
| Rope No | Ø (mm) |
|---|---|

Status
LIFT CAR AND PIT
17. 25mm car door gap test on the doorway
18. Car door mechanical lock function
19. 10mm car & landing door gap with door frame
20. Door protection devices function
21. Counterweight runby signage
Status
23. Buffer condition (e.g. proper fixing, oil level, oil leak, aging polyurethane buffer, cracked or corroded spring)
24. All emergency-stop switches (e.g. machine room, hoist way, car top, lift pit)
25. Overspeed governor rope tension sheave condition
26. Lift pit ladder (e.g. accessibility, condition)
27. Lift pit cleanliness
28. Ventilation / illumination requirements (e.g. machinery space, car top, lift pit)
Full Load Test of Traction Lifts Only
Status = Satisfactory (S); Not Satisfactory (NS); Not Applicable (NA)
ADDITIONAL CHECKS/TESTS FOR FULL LOAD
Machine room / Head room
Status
30. Manual rescue function (electrical / mechanical)
Status
Hoist way
32. Overspeed governor and safety gear function at full load (refer to item 6)
Lift Car and Pit
33. Function of over-load weighing device, its alarm and indicator
Any other observations (to be used for both full and/or no load test):
I, as the supervising SPE, declare that:
