GENERAL PERMIT (7 Day PTW)

Section 1: Pre-Operation Details

This permit is valid for 7 days from time of approval. Application shall be submitted at least 1 day in advance.

Work Description

I have checked and confirmed that all reasonable and practicable measures and requirements governed by the WSH Act and its Regulations have been complied with.

Prior to the commencement and during the course of work, I further undertake all necessary measures and precautions to eliminate all possible risks and hazards posed and carry out the work safely in accordance with the WSH Act and its regulations .

I have conducted the Risk Assessment and Safe Work Procedures briefings to workers involved in the works. I fully understand the nature of work and hazards associated with the work.

I will supervise the above work and undertake to stop/cease the operation should there be any unsafe condition or potential hazardous situation arised during the cause of work.

I have inspected the above and confirmed that reasonable measures are taken to eliminate all possible risks/hazards and the safety measures in place are in good order.

I have also inspected the work area and its surroundings have ensured that all possible risks and hazards have been identified and eliminated or minimised to a reasonable practicable manner for the intended work to be carried out.

The said location is therefore safe to work subject to fulfilment and compliance of the requirements
