Permit to Work at Height

Section 2. Work at Height Checklist

1. Entry and exit points identified for the work at height and where required alternate entry and exit points identified where the work will interfere with normal operations.

2. Immediate area affected by the work at height has been barricaded and signs installed.

3. Drop zone established, barricaded and signs installed.

6. A safety observer has been appointed where required

7. Work at height access equipment has been inspected and has a current inspection tag attached (i.e. EWP, man box, etc.).

8. Work at height safety equipment has been inspected and has a current inspection tag attached.

9. Permanent anchor points are suitable for the work activity, have an information plate, have been inspected and have a current test tag attached.

10. Are temporary anchor points required?
