APPLICATION – To be completed by Supervisor carrying out the work

THIS PERMIT IS VALID FOR 12 HOURS FROM THE TIME OF APPROVAL. APPLICATION MUST BE SUBMITTED AT LEAST ONE DAY IN ADVANCE.

Cordon off or barricade work area

Provide warning signs

Adequate lighting

Provide ventilation

Standby fire watchman

Install flashback arrestors

All workers involved are briefed by the Supervisor In-charge

No incompatible work is allowed e.g. flushing of Chemicals

Provide fire blanket to protect equipment and “live” chemical pipe

Mount gas cylinders on trolley

Area free from combustible materials and gases

Fire extinguisher / fire hose standby in serviceable condition
