Items to Check

1. Operators attended the Safety Orientation Briefing conducted by the Gondola Supervisor

2. Operators are authorized to use the Gondola

4. Ground floor barricaded

5. Minimum of 2 warning signboards placed at strategic locations

3. Operators have been briefed on the safe work procedure for the work to be carried out

6. Signboards are prominently displayed indicating “Safe Working Load (maximum 1000kg)” and “Maximum allowable number of operators (not more than 3 persons)”.

7. Appropriate PPE issued to workers include vertical lifeline, safety harness and fall arrestor for every individual.

8. Workers have been taught on the use of their PPE and the associated safety provision.

9. Proper access provided for the gondolas and MCWP at ground or any other level.

10. Control box in the Gondola covered and locked

11. Two suspended wire ropes and two safety wire ropes provided for the cradle

12. The wire ropes used to suspend the cradle kept tight and vertical

13. Lifelines are strong enough to carry man load.

14. The tension weights properly secured to the safety wire ropes above ground level

Gondola Operator Daily Checklist

Items to Check

1. Operated by trained and authorized personnel only. He must not be under the influence of alcohol, drugs or medicine and must remain sober at all times.

2. Full-body safety harness with dual lanyard & shock absorber available for use when on board platform / deck.

3. Safety provision of Lifeline ( if required ), harness, rope grabs, SWL / Max person signboard, toeboards, barricades, surrounding area effectively cordon off.

4. LP certificate not expired i.e. not more than 6 months from test / inspection date.

5.Tires not severely worn or torn & have adequate pressure.

6.Guardrails & work platform properly secured & not damaged (e.g. cracked welds, loose / missing parts).

7. Slide-out extension platform / deck can be secured by pins and all fasteners properly tightened.

8. Hydraulic fluid within safe level (check only with platform / deck fully lowered).

9. Battery fluid within safe level

10. Operate from chassis / base (not platform / deck) to confirm warning devices (i.e. horn, lowering alarm) working.

11. Operate from chassis / base to confirm no leakage from hydraulic cylinders & no missing / loose parts.

12. All controls at chassis / base & platform / deck can function. (NOTE: return all controls to neutral after check)

13. Check that nothing is obstructing the travelling path, provide ground guide and check that access is sufficiently levelled and stable for travel, i.e. no open side, drop, floor opening etc.

MCWP Operator Daily Checklist

Items to Check

1. It can be operated by trained and authorized personnel only. He must not be under the influence of alcohol, drugs or medicine and must remain sober at all times.

2. Safety provision and PPE checked and in good condition. Lifeline, harness, rope grabs, SWL / Max person signboard, toeboards, platforms ingress & egress, area below effectively cordon off.

3. Parts and components are check for deformation, defects & corrosion at, whole of MCP, racks & mast, bolts & nuts, winches, welded joints, tiebacks, base mounting, telescopic extension etc.

4. Checks base mounting and tie back brackets are firm, stable & secured, no cracks to structural components where it mounts.

5. Conduct functional check that the MCP is functioning as per normal including the telescopic extension and all safety devices like brakes, limit switches, emergency stop button, indicator lights etc.

6. Check that nothing is obstructing the travelling path, upwards and downwards.

7. Check that no other works are being carried out below and above the MCP at any point of time.

8. Check for good housekeeping within the MCP to prevent falling object hazard and that is not overloaded at any point of time.

9. Check that all electrical connection is not loosen, cables in good condition and wtihout risk of entanglement and no signs of burnt.

10. Check that there is no unauthorized extension of work platform/access.
